  • With your consent, yes. We value collaborative care, especially when trauma or complex mental health concerns are involved.

    1. Complete the contact form

    This form helps us get a clear picture of what you’re looking for and how we can best support you. The questions you answer upfront make communication smoother and more efficient from the start.


    After you submit the form, you’ll hear from Autumn, our office manager, within two business days.

    2. Verify your insurance coverage

    Autumn will guide you through verifying your insurance benefits. We’ll send you a short list of specific questions to ask your insurance company.

    All you need to do is call, ask those questions, and write down the answers. This step helps us understand your coverage ahead of time so there are no surprise bills or denied claims later.

    3. Fill out the intake forms

    Once your insurance information is confirmed and sent back to Autumn, you’ll receive access to your intake forms.

    These help your dietitian prepare for your care and understand your needs before your first session. But also include practice policies for your to read over.

    4. Schedule an assessment with your dietitian

    At the same time you receive your intake forms, you’ll be able to schedule your initial assessment.

    Your dietitian will reach out to you directly by email to coordinate scheduling and next steps.
